OR b) As a member of team of Cardiologists / Hospital / Institution. 2) If registering as an individual then simply go to link <Request As Individual> given below and fill up the form. 3) If registering as a member of team of Cardiologists / Hospital / Institution. Then carefully read following points before proceeding further: - Hospital / institution will have to nominate an individual as team leader. - The hospital / Cath lab can have a username and password and the responsibility will be of team leader. - In case there are more than one unit / team in the hospital and they prefer to enter data separately; every unit head / team leader may take a separate registration (under the same hospital name) and give the details of his / her team. - The contact details in the registration form will both be of the concerned hospital and the team leader. - If there is any change in constitution of the team or contact details of members of team, the same should be communicated to us at the earliest & the responsibility for doing so rests with the hospital / team leader. - It will also be the responsibility of institution that if any doctor of the team has registered himself as an individual then due care may be taken that his data is not be duplicated in the hospital figures.
